How to Get Started

Select Request an Appointment below and choose the date and time that fits your needs.

Before Your First Appointment

New patients will be asked to complete registration forms, consent documents, clinical questionnaires, and insurance or payment information before the appointment.

Please provide an accurate list of your medications and, when available, information about previous diagnoses, treatment, hospitalizations, laboratory testing, and medication trials.

For adolescent patients, a parent or legal guardian will generally need to participate in registration, consent, and portions of the evaluation.

The Initial Consultation

The first appointment is a psychiatric consultation and evaluation. It allows the provider to understand your concerns and determine whether outpatient treatment through R&K Wellness Group is clinically appropriate.

The consultation does not guarantee:

  • A particular diagnosis

  • A prescription or medication change

  • Prescription of a controlled medication

  • Completion of forms or disability documentation

  • Establishment of an ongoing treatment relationship

When the practice and patient agree to continue, an individualized treatment and follow-up plan will be discussed.

Insurance and Self-Pay

Insurance participation and self-pay fees are listed on our Insurance & Fees page. Coverage should be confirmed before your appointment.

Urgent and Emergency Needs

R&K Wellness Group does not provide emergency or crisis services.

Do not use the appointment form, email, telephone, text messaging, or patient portal for an emergency.

Call 911 or go to the nearest emergency department if there is an immediate danger or medical emergency. For mental health or substance-use crisis support, call or text 988.
