Self-Pay / Out-of-Pocket Rates

Psychiatric Initial Evaluation — $200
Comprehensive evaluation of your concerns, symptoms, medical and psychiatric history, current medications, and treatment goals. When appropriate, treatment recommendations and medication management are discussed.

Follow-Up Medication Management — $100
Follow-up visits focus on treatment response, medication effectiveness, side effects, symptom changes, and adjustments to your treatment plan as clinically appropriate.

Before Your Appointment

Please submit accurate insurance information before your appointment, including photographs of the front and back of your insurance card when requested.

Any required copayment, deductible, or coinsurance may be collected at the time of service.

Insurance coverage does not guarantee that a specific medication, evaluation, laboratory test, form, or treatment recommendation will be covered.

Billing Questions

Please contact the practice before your appointment if you have questions about insurance participation or billing procedures.

Final decisions regarding eligibility, coverage, authorization, medical necessity, and payment are made by the patient’s insurance plan.
